Online Order Instructions

To order one or more "Uh-oh! Where'd My Name Go?" books, do the following:

  1. Complete the order form on the right and then click Continue.
  2. Provide the child's personal information for each book you order.
  3. Complete payment for the order.
  4. Upload or mail in the child's photograph.
  5. Repeat the process for each additional shipping address.
  6. Allow four to six weeks for delivery.

You may pay by credit card using PayPal or personal check or money order.  We recommend that you pay by credit card. PayPal is safe and secure.
